Treasure Chest of Hope - Donation Form

If you wish to make a donation to our 11th Annual Treasure Chest of Hope, please print and fill in the following donation form and fax it in to us. We welcome donations of cash, or items for our Live Auction, Silent Auction and the Balloon Raffle.

Charitable tax receipts will be provided for cash donations.

If you would like a charitable tax receipt for in-kind donations you will be required to provide third party documentation of the value of the donated item. This may take the form of a newspaper advertisement, value confirmation, fee for service schedules, or if the item was newly purchased, a receipt.
